Register with the practice?

We welcome new patients from within our catchment area. You will need to bring two forms of identity. One will need to be photo ID, eg passport or driving licence and the other needs to be a recent utility bill or bank statement confirming your address. When you register you will be asked to complete a short questionnaire. Patients may also be asked to make an appointment with a doctor to discuss any ongoing problems and current medication. Medical notes may take several weeks to be transferred from your previous GP, so this registration questionnaire is most important if we are to give the best patient care in the meantime.

Under the new GMS Contract, patients are now registered with the practice rather than an individual doctor. However, a patient is entitled to express a preference for the doctor they wish to see.
